The Canadian Citizenship Certificate Application (CIT 0001) form does not have enough space to document multiple generations of family citizenship information. This website offers fillable PDF addendum templates for Section 9 (great-grandparent information).

These forms are available for download in both English and French, covering applicant generations G3 through G20.

🔢 Understanding Generation Numbers

A "G-number" (generation number) tells you how many generations back your Canadian ancestor is from you. G0 refers to the last person in your family line who was born or naturalized in Canada. For more detail, see the r/CanadianCitizenship Start Here Guide.

If an applicant is G3 (their great-grandparent was the last Canadian-born person in their line), they need to document their great-grandparents to prove their citizenship claim. Use the G03 addendum template, which has 2 pages.

If an applicant is G5 (their 3× great-grandparent was the last Canadian-born person in their line), they need to document their great-grandparents, great-great-grandparents, and great-great-great-grandparents to prove their citizenship claim. Use the G05 addendum template, which has 6 pages.

📋 How to Use These Documents

Step 1: Determine Your Generation Number

Find the last person in your family line who was born or naturalized in Canada. They are your G0. From 0, count forward by generation to yourself. That is your own generation number.

Step 2: Download the Appropriate Document

Select the document that matches your generation number and preferred language (English or French).

Step 3: Complete the Form

The PDFs are editable. You can fill them out digitally or print and complete by hand. If you don't know certain information, enter "unknown". If information doesn't apply, enter "not applicable" or "NA".

Step 4: Submit with Your CIT 0001 Application

Include the completed addendum template as part of your Canadian citizenship certificate application submission.

❓ Frequently Asked Questions

I am applying together with a group of family members. Do we all need addendums?

The CIT 0001 instructions for Section 9 say to include great-grandparent information on extra pages. You should include an addendum for each generation that needs one. However, people who share the same ancestors in a group application can use the same completed addendum. For example, two G4 brothers have the same great-grandparent and great-great-grandparent information. Just print two copies and include one with each person's CIT 0001 form.

Should I insert the addendum into the middle of the CIT 0001 form, or add it at the end?

An approval was reported on 2026-03-27 where they put the addendum pages in the middle right after section 9. Other applications have put the addendum right after the CIT 0001. The placement is not specified in the section 9 instructions. Use your best judgement and focus on providing accurate information.

I have a question on the addendums, the CIT 0001, or something else about this process. Can you help me?

The best known resources are linked at the bottom of this page. The author is not qualified to respond to direct messages about applications, but there's a whole community of people who can help on Reddit or Facebook.

What happened to the Google Drive? Are these the same templates?

Yes, they are the same ones. These templates were generated with Python code. The author wanted to make sure that the code was preserved in case the templates needed to be changed or regenerated. To do that, the code and templates were added to GitHub, and a no-cost GitHub Pages website was added to make the addendum templates self-explanatory. Google Drive was a quick fix to get the templates out into the wild, and the GitHub work happened about a month later.

These forms look "too" official. Why should I use them?

The first known approval using these templates happened on 2026-03-27.

The CIT 0001 instructions for Section 9 great-grandparent info (below the signature area) state: "Provide the same information that is requested in Section 9." Read that section (and all sections) in full and use your best judgement.

These forms were shared by a volunteer and are free for anyone to use. You are welcome to modify them or not use them at all. That said, there have been reports of applicants reaching the AOR stage while using these templates. As of March 2026, 3 users have reported receiving their AOR. No one has reported having their application rejected because of these forms.
